Position Overview:
We are seeking a dedicated and compassionate Registered Nurse to join our Home Health team in Oakdale. This role involves providing exceptional nursing care to patients in their homes, ensuring they receive the highest standard of medical treatment and support. You will be an integral part of a multidisciplinary team, focused on improving patient outcomes and enhancing the quality of life for those in need of home healthcare services.

Key Responsibilities:

• Conduct comprehensive patient assessments to develop individualized care plans.
• Administer medications and treatments as prescribed by the healthcare provider.
• Monitor patient conditions and communicate changes to the healthcare team.
• Educate patients and their families on health management and disease prevention.
• Collaborate with other healthcare professionals to coordinate patient care and ensure continuity of services.
• Maintain accurate and timely documentation of patient care activities.

Qualifications:

• Current and valid Registered Nurse (RN) license in the state of practice.
• Minimum of one year of nursing experience.
• Require Home Health Experience
• Strong clinical skills with a focus on patient-centered care.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Basic life support (BLS) certification required.

Working Environment:
This position involves traveling to patient homes within Oakdale and surrounding areas. The ideal candidate will be comfortable working independently in different home settings and adaptable to changing circumstances.
