• Initiate the plan of care based on identified needs
• Provide treatment based on physicians orders and plan of treatment
• Provide specific care to patients with IV, wounds, complex medications and other equipment
• Use critical thinking skills to assess patient's condition and determine appropriate interventions.
• Recognize changes in a patient's status and reports appropriately.
• Provide education specific to diagnosis and symptom management needs

Requirements:

• Current RN license in the state of Tennessee
• Experience in post-acute care is a huge plus
• Should have at least one (1) year of recent nursing experience and 1 year of home health experience preferred
• Knowledge of OASIS is preferred. Must be very detail oriented in documentation.
• Current CPR and TB Screening prior to patient visits
• Excellent driving record

Ideal candidate will have a strong clinical background to aid in the assessment and plan of care for patients. Working knowledge of Start of Care, Resumption of Care, etc., with the ability to complete all documents associated with these types of visits. This position requires travel between patients within the assigned area.
