Job Description

RN - Care Manager (Remote MA residents only)

Contract Details:

  • Pay: Up to $55 per hour
  • Employment Type: Full-time

Important Note: Candidates must be currently living in Massachusetts in order to be considered for this role

Key Responsibilities:

  • Perform clinical assessments and interpret medical information to create individualized care plans
  • Provide patient education, coaching, and support for managing chronic conditions
  • Coordinate care across multidisciplinary teams, including physicians, specialists, and social services
  • Monitor patient progress, adjust care plans, and ensure timely interventions
  • Address social determinants of health and reduce care disparities where possible

Required Experience:

  • Active RN licensure in Massachusetts
  • 2+ years of clinical experience in inpatient, outpatient, or similar settings
  • Proven ability to work independently and collaboratively in a fast-paced environment

Nice-to-Have Experience:

  • Case Management certification (CCM) or equivalent credential
  • Experience in diabetes education or health coaching (e.g., Wellcoach certification)
  • Prior experience managing medication plans and coordinating care teams
